<br /> <br /> <br /> Scholarships <br /> All fitness classes and parks and recreation programs have scholarships available. Membership fees do not qualify <br /> under the scholarship policy. For more scholarship information and fees, call 651-638-2121. <br /> <br /> insurance Benefit <br /> The following insurance plans are available for qualifying members: Blue Cross Blue Shield; UCARE; Definity; <br /> Medica and Health Partners. Select memberships over $20 a month qualify under HP & Medica guidelines; no <br /> additional discounts apply. Fitness Center member participants that qualify for an insurance benefit must fill out an <br /> eligibility form and sign an annual member agreement. Participants are responsible to track their progress and <br /> make arrangements for work out times during building closures, and personal leave such as vacation or medical. If <br /> you qualify with your insurance, this program will allow you to redeem a credit for up to $20 per month which would <br /> be applied upon renewing your annual membership. Please enjoy insurance services we currently provide or join <br /> us with our already low rates. <br /> Insurance Membership Non-Renewal/Cancellation <br /> Members who take advantage of the insurance benefit are required to fill out a 30 day non-renewal cancellation <br /> form for the insurance credit earned. <br /> Enrollment Fee <br /> A joiner fee applies to various memberships. <br /> Membership Renewals <br /> Patrons may renew their memberships no more than 30 days in advance. The new membership will begin after <br /> your current membership expires. The current membership classification at the time of purchase will be upheld. <br /> Refunds or Transfers <br /> All memberships and personal training are non-refundable and non-transferable from date of purchase. <br /> Membership Upgrade <br /> Patrons may choose to upgrade their membership at anytime, but may not downgrade. Membership upgrades can <br /> only be changed from the date requested. Your current membership will be prorated and withdrawn from the <br /> original start date. You may use the remaining balance to pay the difference. <br /> Membership Holds <br /> Holds provide Family Service Center members with the privilege to optimize use of the FSC. A member may place <br /> a hold on an individual or family, annual membership (excluding the 3 month and 9 month extension) one time <br /> during their annual membership cycle. The hold may be placed for no less than two weeks and no more than 5 <br /> months per annual membership. You may not place individuals on hold within a family membership; the entire <br /> family is put on the hold. All holds must be approved by the Recreation Supervisor. If the hold request is denied, <br /> you will receive a follow-up call. <br /> Members must apply for the hold 48 hours prior to their leave date, additional fees may apply. A specific start <br /> (leave) and end date (return) for the hold must be provided at the time of request to ensure accuracy in our record <br /> keeping. The hold dates may not be changed once it has been placed on a membership. If you should return prior <br /> to the requested end date you must purchase a punch pass or day pass until your hold ends. You may not receive <br /> insurance visit accrual until your hold ends. <br /> A pro-rated membership refund minus a processing fee will be given for medical or military reasons (papers <br /> provided) in which an end date (return time) is unknown. When you are able to return we encourage you to join the <br /> Family Service Center. <br /> Punch Passes <br /> A twelve punch pass is available for use of Fitness Center, Eagles Nest, and Gymnasium as in the form of a <br /> computerized scan card. A five punch pass may be available at select times throughout the year. Punch passes <br /> are only valid for the person(s) to whom it is issued. Punch passes are non-refundable and non-transferable, but <br /> they do not expire. Paper punch passes do have firm expiration dates and are non-refundable and non- <br /> replaceable if lost. <br /> <br /> <br /> <br /> 2 <br />
