We have <br />already completed some parts of the form(s) from OUr <br />records. Please complete the remaining parts in <br />accordance with the following instructions: <br /> <br />I <br /> <br />List only those costs which were actually <br />incurred by your law enforcement agency. <br />If all or part of an officer's salary was <br />subsidized during the training periodt <br />state only the part of the salary that was <br />paid out of your local budget. <br /> <br />The applicant is the local official who is <br />authorized to apply for reimbursement funds. <br />The applicant's signature must be notarized. <br /> <br />J <br /> <br />The applicant must have been designated by <br />a resolution of the local governing body. <br />This resolution must have been passed <br />within one year of the date of application, <br />unless the applicant is the same official <br />who was named when the city or county last <br />applied for reimbursement funds. If you <br />do not have a copy of a past resolution <br />available, my staff can look up the date <br />of the last resolution and the official who <br />was named ae applicant. <br /> <br />When an officer's salary was SUbsidized~" <br />reimbursement for that officer's traini is $240.
