Dear Chief of Police or Sheriff: You may use the enclosed form(s) to apply for partial reimbursement of the costs that your law enforcement agency incurred in sending the named peace officer(e) to a basic law enforcement training course during the fiscal year which ends on June 30, 1980. We have already completed some parts of the form(s) from OUr records. Please complete the remaining parts in accordance with the following instructions: I List only those costs which were actually incurred by your law enforcement agency. If all or part of an officer's salary was subsidized during the training periodt state only the part of the salary that was paid out of your local budget. The applicant is the local official who is authorized to apply for reimbursement funds. The applicant's signature must be notarized. J The applicant must have been designated by a resolution of the local governing body. This resolution must have been passed within one year of the date of application, unless the applicant is the same official who was named when the city or county last applied for reimbursement funds. If you do not have a copy of a past resolution available, my staff can look up the date of the last resolution and the official who was named ae applicant. When an officer's salary was SUbsidized~" reimbursement for that officer's traini is $240. In all other cases, equal shares will be pai eligible officer who was trained during the past fiscal year.
