RESOLUTION NO <br />10-033 <br />CITY OF NEW BRIGHTON <br />COUNTY OF RAMSEY <br />STATE OF MINNESOTA <br />ASOLUTION APPROVING THE AGREEMENT BETWEEN HEALTH PARTNERS AND <br />THE CITY OF NEW BRIGHTON. <br />the Family Service Center of New Brighton offers the HealthPartners incentive <br />program to its members, and <br />this program is called the HealthPartners Frequent Fitness Program, and <br />in this program, HealthPartners agrees to offer a monetary incentive to certain eligible <br />persons who complete a given number of monthly workout sessions at the FSC, and <br />HealthPartners chose to transfer to Vanco processing service for club member <br />reimbursements, thus requiring the City of New Brighton to sign a new contract, and <br />the Family Service Center currently uses Vanco processing service for Blue Cross Blue <br />Shield, United Health and UCare reimbursement programs, and <br />by offering these services, we maintain a strong retention program for our members, and <br />HealthPartners is an incentive program to Family Service Center members with twelve or <br />more visits each month, earning back $20 per month toward membership dues. <br />, THEREFORE, BE rf RESOLVED by the City Council of the City of New Brighton to hereby <br />the resolution approving the agreement between HealthPartners and the City of New Brighton. <br />this 11 th day of May, 2010. <br />Dave jacobWn, Mayor <br />Dean R.
