Instructions for Completing the PREP Form <br />Purpose - <br />Performance management in the City of New Brighton is a continuous communication process in <br />which (1) expected levels of performance are defined and communicated, (2) performance is <br />planned, maintained, and improved, (3) actual performance is compared to expected levels, and <br />(4) accomplishments and areas for improvement are recognized. The performance review may <br />also be used in management decisions regarding assignments, career development, reassignment <br />or corrective action. <br />The purpose of this annual performance evaluation is to: <br />■ Agree to a common understanding of the department and individual goals; <br />■ Provide objective criteria for personnel performance on a standard basis; <br />■ Provide feedback to employers and employees that enhance performance, efficiency, and <br />effectiveness; <br />■ To identify employee's strengths and areas for improvement as they related to the <br />employee's position; and <br />■ To reinforce supervisor /employee communications. <br />Instructions- <br />1. <br />Complete the evaluation at least two weeks prior to the deadline issued by the <br />City Manager. <br />2. <br />Collect and evaluate information related to employee performance to be <br />considered at part of the review. <br />3. <br />Indicate your rating in each section of the evaluation. Rate each category <br />separately being careful not to be influenced by your overall judgment of the <br />employee's performance. <br />4. <br />Use the "Comments" section to explain your rating, note examples, and identify <br />goals and expectations for the next review period. <br />5. <br />Ratings from each category should be weighted and tabulated to determine the <br />overall performance score. <br />6. <br />Discuss the evaluation with the employee. <br />7. <br />Employee and Supervisor should sign and date the evaluation where indicated. <br />8. <br />Submit the completed Evaluation and PREP form to the Assistant to the City <br />Manager for review. <br />9. <br />The completed Evaluation and PREP form is submitted to the City Manager <br />review. <br />10. <br />Once the PREP form is finalized a copy of the PREP will be provided to the <br />employee for their personal records. <br />
