Audio: <br />Date and time of digital video inspection, operator name and name of adjacent street <br />Verbal confirmation of upstream and downstream manhole numbers and camera direction in relation to flow <br />Verbal or electronic description of pipe size, type, and pipe joint length <br />Verbal description of 2nd attempts, 3rd attempts, reverses, etc <br />A narrative description of the observable situation encountered as the camera travels through the sewer, including estimated <br />inflow rates, clock position, joint problems and major cracks <br />Written Report : <br />PipeTech software program — report printed in top view option <br />Date, time, city, street, survey direction, pipe ID and name of operator <br />Start Manhole and end Manhole with upstream and downstream manholes listed <br />Location and description of each point of leakage <br />Location and type of each service connection <br />List active or abandoned/capped service connections and estimated current flow <br />Location of any damaged sections, nature of damage, and location with respect to clock position <br />Deflection in alignment of grade of pipe <br />Pipe diameter, pipe material, section length, direction of viewing <br />List a change in pipe — describe the material <br />Abandoned survey — list cause, e.g. roots, blockage, sag, grease, etc. List total length of line vs actual length of line <br />surveyed on report (wheel walk to measure total length of tine, if survey abandoned). List 2"d attempts, 3rd attempts. and <br />reverses. <br />Type of surface, e.g. aggregate, concrete, bituminous, turf, etc <br />Furnish a list of incomplete lines <br />Speed of the camera through the pipe shall not exceed the speed sufficient to permit visual analysis of infiltration, structural <br />damage, line potential and grade problems. <br />Viewing software must have fast forward, drag bar, and allow viewer to click ahead to an event on the report. <br />See attached map for approximate locations of work. <br />TOTAL cost of televising 92,500 feet of sanitary sewers: $ , qt <br />All work is to be completed by October 30, 2015. There will be a 10% penalty if work and video submission is not <br />complete by due date. Failure to meet this deadline may result in loss of future quoting opportunities.
