<br />(6) An executed contract for septic service management or other proof of <br />adequate septic service management; <br />(7) An affidavit that the applicant has provided notice to adjacent property <br />owners and residents of the application for a temporary health care dwelling <br />license; <br />(8) A site map showing the location of the temporary family health care dwelling <br />and all distances to other structures on the lot; <br />(9) The location of the temporary family health care dwelling shall meet all <br />setback and building coverage requirements established by the underlying <br />zoning district designation. <br />(10) The location of the temporary family health care dwelling shall be such <br />that septic services and emergency vehicles and personnel can gain access in <br />a safe and timely manner. <br />(11) The number of occupants of a temporary family health care dwelling shall <br />be limited to one and those that are providing care to the occupant, but not to <br />exceed three total. <br />(12) Only one temporary family health care dwelling is allowed per lot. <br />(13) Submission of proof of a general liability insurance policy covering the <br />temporary family health care dwelling. <br /> <br />Section 15-805. License Term. <br /> <br />A license shall only be valid for six months. The applicant may renew the license <br />once for an additional six months, or request a partial renewal on a month-to- <br />month basis not to exceed six months. At least onetwo years shall separate <br />multiple license applications and effective dates for a single property. <br /> <br />Section 15-806. Inspection. <br /> <br />Following issuance or renewal of a temporary family health care dwelling license <br />and occupancy of the temporary family health care dwelling, the City’s Fire <br />Marshal, Code Compliance Inspector, and Building Official shall inspect the <br />dwelling to ensure compliance with this Article. The applicant shall request <br />required inspection no later than three business days following initial occupancy. <br />Additional inspections by the City’s Fire Marshal, Code Compliance Inspector, <br />and/or Building Official shall be allowed, upon 24 hours’ notice, if a complaint is <br />received pertaining to the temporary family health care dwelling. <br /> <br />Section 15-807. Revocation of License. <br /> <br />The temporary family health care dwelling license shall be revoked if the licensee <br />violates any requirement of this Article. <br /> <br />Section 15-808. Fee. <br /> <br />A license fee shall be due at the time of application and in an amount as <br />determined by the City’s Fee Schedule. Reduced fees will be required for a <br />renewal license. <br />
