<br /> <br />5 <br /> <br />3. The circle drive off of Black Oak Drive shall be a one-way to the north, and a “do not <br />enter” sign should be placed at the northern entry point. <br />4. Minor changes as requested by public safety to improve emergency vehicle turning <br />movements shall be incorporated into the plans as necessary. <br />5. Drainage and utility easements (or others), as required by the City Engineer, shall be <br />provided on the future final plat. <br />6. A new (minimum) 6 foot tall fence shall be installed between this development and the <br />residential units to the NE. Materials, style, location, and ownership of the fence is to be <br />worked out with the neighboring residents prior to final installation. <br />7. The lighting plan shall be amended (if necessary) to meet all minimum required <br />standards prior to final plat submittal. <br />8. The Benedictine Health Center shall share in maintenance costs for ponding if the <br />adjacent HOA pond is impacted by the proposed project. <br />9. Damage to surrounding properties as a result of construction shall be dealt with by the <br />responsible contractor <br />10. All utilities (i.e. telephone, electric, gas service lines, etc) are to be placed underground. <br />11. Should parking problems arise post development, underground spaces assigned to <br />assisted living and/or independent living units shall reallocated as necessary to address <br />the problem. <br />12. The applicants shall erect a main entry monument sign near the new entrance off 27th <br />Avenue NW. The sign should clearly designate this entry point as the main entrance for <br />all visitors and staff. Final design to be determined with final PRD plans. <br />13. The existing sidewalk leading to Linden Drive shall be relocated (or a new sidewalk shall <br />be added) to line up with and provide access to the existing trail system to the north <br />leading into Innsbruck Park. <br />14. The applicant shall work with the City Forester on final planting locations and mix shown <br />on the landscaping plan prior to submission of final plat. <br />15. The applicant shall work with the City Engineer regarding proper tree placement near <br />the ROW of Linden Drive. <br />16. All trash and recycling enclosures must be constructed of sturdy, opaque materials (with <br />trash and recycling receptacles screened from view) that are in harmony with <br />architecture and materials of the main building. <br />17. A final plat shall be submitted combining all parcels into a single parcel. <br />18. Approval of this Preliminary Planned Residential development shall remain valid for 180 <br />days from the date of approval. One extension of up to an additional 180 days may be <br />approved by the City Council thereafter. <br /> <br />
