<br /> <br /> <br />REQUEST FOR CITY COUNCIL CONSIDERATION <br /> <br />ITEM DESCRIPTION: CONSIDERATION OF CHANGE ORDER 1 AND FINAL PAYMENT, PARTIAL <br />PAYMENT 2 FOR CITY PROJECT 19‐3, 2019 PAVEMENT MANAGEMENT PROGRAM <br />DEPARTMENT HEAD’S APPROVAL: Craig Schlichting – Director of Community Assets and <br />Development <br />CITY MANAGER APPROVAL: <br /> <br /> <br />No comments to supplement this report _______ Comments attached ______ <br /> <br />Recommendation: <br />To approve Change Order 1 and Final Payment, Partial Payment 2 for City Project 19‐3, 2019 <br />Pavement Management Program to T.A. Schifsky and Sons, Inc. in the amount of $5,549.05. <br /> <br />Legislative History: <br />June 25, 2019 City Council approved City Project 19‐3, 2019 Pavement Management <br />Program and authorized staff to solicit quotes. <br /> <br />August 13, 2019 City Council approved quotes and awarded a contract to T.A. Schifsky and <br />Sons, Inc. <br /> <br />Financial Impact: <br />The attached Change Order 1 addresses extra costs in the amount of $3,625.00 for actual <br />quantities installed in the field. The original contract amount, which was based on estimated <br />quantities, was $107,356.10. Please see attached payment and Change Order for details. <br /> <br />Explanation: This project consisted of the bituminous pavement removal and replacement, curb <br />and gutter repairs, and parking lot striping at the following locations: <br /> <br /> Freedom Park Parking Lot (west of Pike Lake Education Center entrance) <br /> Water Treatment Plant #3 (well 11 and well 14) <br /> <br />Costs and Funding: <br />This project will be funded from Fund 606 and Fund 206.
