Director of Public Works <dv\1..\) <br /> .. <br />CITY MANAGER'S APPROV AL: <br />No comments to supplement this r~p?_ Comments attached <br />- <br /> <br />~ <br /> <br />Recommendation: To approve Change Order No.1 in the amount of $1,800 and Partial Pay <br />Application No.1 in the amount of $46,485.00 completed on Project 06-4, Well 8 & 11 Filter <br />Rehabilitation and Chemical Feed Modifications. <br /> <br />Legislative History: Project 06-4, Well 8 & 11 Filter Rehabilitation and Chemical Feed <br />Modifications, was authorized by the Council on December 13, 2005 to reduce radium levels at <br />Water Treatment Plants 3 and 3A. One of the necessary components of Project 06-4 includes the <br />rehabilitation of Well No.8. A construction contract for the rehabilitation of Well NO.8 was <br />awarded to LeadCon, Inc. on February 28, 2006. <br /> <br />. Financial Impact: The original contract was awarded in the amount of $49,850. The attached <br />Change Order No.1 addresses the increase in the contract amount of $1,800. <br /> <br />Explanation: This change order is for the installation of an additional manway access hatch on <br />the air chamber on filter #8. There is a header pipe inside the chamber with very little clearance <br />to get under and work on the backside of it. A thin person can get under it, but the blast hood for <br />sandblasting would not. Essentially, repainting this filter would not be possible without this <br />access hatch. <br /> <br />LeadCon Inc. has satisfactorily completed the construction work through July 1, 2006 in <br />accordance with the contract plans and specifications. Final payment for this project will be <br />made once the proper amount of Liquidated Damages is determined since the project was not <br />completed on schedule.
