-Employees are considered anyone who works full time or permanent part time. Permanent <br />part time is an employee who is on the company payroll 12 months a year and works a <br />minimum of 20 hours per week. To qualify for a family memberships an employee must be ' <br />~~~ the head of household (parents or guardian). <br />'~=~ -Each company is required to assign one employee to administer their companies membership <br />program. This person is responsible for verifying employment, accumulating and sending in <br />rosters and being the contact for Family Service Center staff when needed. <br />-The roster must consist of all employees wishing to participate and; <br />1. Employee full name <br />2. Home address <br />3. Work and home phone numbers <br />4. Birth date -day, month, year <br />-For family memberships please include: <br />1. First name of all family members <br />2. Birth dates of all family members <br />-Each participating company is required to provide full payment for each employee <br />registering. <br />-After payment is received, employees may come in to have their picture taken for the I.D. <br />card. All family members over the age of two must have a picture I.D. card. <br />FACILITY RENTAL RATES <br />City Sponsored programs will consist of open gym, open basketball, open volleyball, open <br />badminton, leagues, lessons and special events. It will be the policy of the Family Service <br />Center to provide at least 1 space for general use (open gym) at all times... The gym can be <br />used for outside rentals by groups for tournaments, leagues and general use based on <br />availability. <br />TEEN LOUNGE <br />The teen lounge will be available for youth grades 6,7,8 under supervision of Northwest <br />Youth and Family Services. Video games, movies, homework or a place to visit is the intent <br />of this area. <br />*A fee for set up and/or a building supervisor may apply. <br />
