The name of the manager, proprietor, or other agent in charge of the business <br />and all information concerning each manager, proprietor, or agent required <br />in #2 above. <br /> <br />5. In the case of all applicants: <br /> <br />Sec. 15-505 <br /> <br />a. <br /> <br />Whether the applicant holds a current tattooing, body piercing, body <br />branding or body painting license from any other governmental unit. <br /> <br />Whether the applicant has previously been denied a tattooing, body piercing, <br />body branding or body painting license from any other governmental unit. <br /> <br />The location of the business premises and the legal description thereof. <br /> <br />b. <br /> <br />c. <br /> <br />d. <br /> <br />Whether all real estate and personal property taxes that are due and payable <br />for the premises to be licensed have been paid, and if not paid, the years and <br />amounts that are unpaid. <br /> <br />Ifthe application is for premises either planned or under construction or <br />undergoing substantial alterations, the application must be accompanied by a <br />set of preliminary plans showing the design of the proposed premises to be <br />licensed. A copy of the plans must be submitted to building, fire and health <br />authority for approval. If the plans of design are on file with the City <br />Building and Inspection Division, plans need not be submitted. <br /> <br />Such other information the city manager may require. <br /> <br />e. <br /> <br />f. <br /> <br />Application Execution. <br /> <br />An application for a license must be signed and sworn to. If the application is that of <br />a natural person, it must be signed and sworn to by such person; ifthat of a <br />corporation, by an officer thereof, if that of a partnership, by one of the general <br />partners; and if that of an unincorporated association, by the manager or managing <br />officer thereof. Falsification of information on a license application requires the <br />denial of a license. <br /> <br />Sec. 15~506 <br /> <br />Application Verification. <br /> <br />An application will be referred to the Director of Public Safety for verification and <br />investigation of the facts set forth in the application and any necessary criminal <br />background checks to assure compliance with this section. <br /> <br />Sec. 15-507 <br /> <br />License Period and License Fees. <br /> <br />The term of the license is the calendar year or the remaining portion thereof. The fee <br />will not be prorated. The fee for a license is set forth by the City Council. The fee <br />must be submitted at the time the application is filed. Fees are non-refundable. <br /> <br />Sec. 15~508 <br /> <br />Persons Ineligible for License. <br /> <br />1.
