Request for unpaid military leave shall be accompanied by a copy of the military orders <br />and shall be made prior to the commencement of the unpaid leave. <br /> <br />Sec. 21-67. Sick Leave. <br /> <br />(1) Purpose. Sick leave shall not be considered as a privilege or vested right which employees <br />may use at their discretion but shall be considered as a type of insurance which shall be allowed <br />in case of necessity and actual illness, legal quarantine or disability of the employee. Use of sick <br />leave sha1l also be allowed in the case of critical illness or death in an employee's immediate <br />family or in order to receive dental or medical care or other sickness preventative measures <br />approved by the employee's supervisor. Critical illness in this case shall be defined as an illness <br />of such a nature that a physician indicates there is the imminent probability of death or that the <br />employee is ordered by a physician to observe and monitor the condition ofthe health of the <br />employee's kin. <br /> <br /> <br />(2) Notice. The employee shall request use of sick leave prior to the beginning ofthe employee's <br />workday. The request shall be made to the employee's immediate supervisor. An employee <br />claiming sick leave may be required to file a signed physician's certificate or sign a statement <br />indicating the nature of the illness. <br /> <br />(3 }-Funeral/Illness in Family. <br /> <br />Employees may use up to 3 days of paid funeral leave to attend the funeral of a member of the <br />employee's immediate family (see definition of immediate family). Up to 2 additional days of <br />sick leave may also be used for attending the funeral ofthe employee's immediate family. <br /> <br />Up to three days of sick leave may also be used for the purpose of attending to the needs of a <br />critically ill member of the employee's immediate family. Critically ill is defined where a <br />physician has indicated that there is the imminent probability of death or that the employee is <br />ordered to observe and monitor the condition ofthe health of the employee's kin. This period of <br />!: sick leave usage may be extended in 30 calendar day increments, up to 90 calendar days, if pre- <br />I approved by the City Manager. This leave shall not be added to leave given under Family <br />Medical Leave Act (FMLA). <br /> <br />(4) Denied. No sick leave shall be allowed for illness, injury, or physical inability resulting from <br />misconduct on the job or excessive use of alcohol or narcotics, except as provided in Section 21- <br />67(1). <br /> <br />(5) Termination. No sick leave benefits of any kind will be granted after termination of <br />employment. <br /> <br />(6) Rate of AccruaL <br />A. Rate of Accrual for Regular Employees. Sick leave shall be accrued by all regular and <br />probationary employees at the rate of eight hours for each calendar month of full-time <br />service. <br />B. U",e of Sick Leave.
