(1) The name and address of the applicant; (2) A list of the equipment whieh he prapase3 t6 tlse proposed to be used in such collection; (3) The place or places to which the garbage and rubbish is to be hauled; (4) The manner in which said garbage is to be disposed of; (5) The portion of the city in which collections are to be made. Section 6. The New Brighton City Code is amended by adding Sections 11-32 through 11-36 as follows: Sec. 11-32. Cate~ories of Licenses. The following categories of haulin~ activities must be separately licensed: {!Y Haulin~ of recyclables. ill Hauling of any non-recyclables and yard wastes from residences. ~ Hauling of any non-recyclables and yard wastes from commercial establishments and/or multiple dwellings. Each application for a license and each license issued shall identify the type of license and the specified vehicles to be used by the licensed hauler. Sec. 11-33. Insurance ReQuired. Before a license is issued, the applicant shall file with the City Clerk evidence that the applicant has in effect public liability insurance for the haulerls business and for all vehicles in at least the sum of $500,000 for injury for one person, $1,000,000 for the injury of two or more persons in the same accident, and $100,000 for property damage. Sec. 11-34. Rate Filing. When the application is submitted, the applicant shall file with the City Clerk a schedule of proposed rates to be charged durinR the licensed period. In the case of service to residences, a minimum of three rate levels for regular service, priced on the basis of volume, will be provided. Every licensee shall provide 14 daysl prior written notification to the City and the licenseels customers of any change in rates to be implemented during the license period. Sec. 11-35.
