<br />(1) The name and address of the applicant; <br /> <br />(2) A list of the equipment whieh he prapase3 t6 tlse proposed to be used <br />in such collection; <br /> <br />(3) The place or places to which the garbage and rubbish is to be hauled; <br /> <br />(4) The manner in which said garbage is to be disposed of; <br /> <br />(5) The portion of the city in which collections are to be made. <br /> <br />Section 6. The New Brighton City Code is amended by adding Sections 11-32 <br />through 11-36 as follows: <br /> <br />Sec. 11-32. Cate~ories of Licenses. <br /> <br />The following categories of haulin~ activities must be separately <br />licensed: <br /> <br />{!Y Haulin~ of recyclables. <br />ill Hauling of any non-recyclables and yard wastes from residences. <br />~ Hauling of any non-recyclables and yard wastes from commercial <br />establishments and/or multiple dwellings. <br /> <br />Each application for a license and each license issued shall identify the type <br />of license and the specified vehicles to be used by the licensed hauler. <br /> <br />Sec. 11-33. Insurance ReQuired. <br /> <br />Before a license is issued, the applicant shall file with the City Clerk <br />evidence that the applicant has in effect public liability insurance for the <br />haulerls business and for all vehicles in at least the sum of $500,000 for <br />injury for one person, $1,000,000 for the injury of two or more persons in the <br />same accident, and $100,000 for property damage. <br /> <br />Sec. 11-34. Rate Filing. <br /> <br />When the application is submitted, the applicant shall file with the <br />City Clerk a schedule of proposed rates to be charged durinR the licensed <br />period. In the case of service to residences, a minimum of three rate levels <br />for regular service, priced on the basis of volume, will be provided. Every <br />licensee shall provide 14 daysl prior written notification to the City and the <br />licenseels customers of any change in rates to be implemented during the <br />license period. <br /> <br />Sec. 11-35. ReQuirements and Restrictions. <br /> <br />All licensees, and any other haulers in the City, shall comply with all <br />of the followinll: reQuirements and restrictions: <br /> <br />4 <br />
