<br />1./ <br /> <br />~ <br /> <br />,-- <br /> <br />RESOLUTION NO. 89-~ <br /> <br />STATE OF MINNESOTA <br />COUNTY OF RAMSEY <br />CITY OF NEW BRIGHTON <br /> <br />RESOLUTION AWARDING THE CONTRACT FOR EMPLOYEE LIFE INSURANCE AND OPTIONAL <br />LIFE INSURANCE <br /> <br />WHEREAS, Minnesota Mututal Life submitted the lowest proposal which <br />provides employee life and optional life insurance; <br /> <br />WHEREAS, the City is desirous of making such insurance available; <br /> <br />NOW, THEREFORE, BE IT RESOLVED, the City Council of the City of New <br />Brighton to hereby adopt the following: <br /> <br />1. The contract is awarded to Minnesota Mutual Life. <br /> <br />2. The terms of the insurance shall be as stated in the <br />specifications as amended by their letter dated September 27, 1989. <br /> <br />3. The monthly premium shall be: <br /> <br />Group Life $.23 per $1000 of coverage <br />Group AD & D $.05 per $1000 of coverage <br />Optional Life <br />Aqe nearest birthday monthly rate per $1000 <br /> under age 35 S .15 <br /> age 35 to 39 $ .17 <br /> age 40 to 44 S .23 <br /> age 45 to 49 $ .36 <br /> age 50 to 54 $ .54 <br /> age 55 to 59 $1.04 <br /> age 60 to 64 $1.09 <br /> age 65 to 69 $2.04 <br /> age 70 to 74 $3.79 <br /> <br />Accidental death and dismemberment is included at a rate of <br />$.04 per $1000. <br /> <br />Adopted this 24th day of October, 1989. <br /> <br /> <br />~ <br />Robert Benk , Mayor <br /> <br />_~ m,0l\tck <br /> <br />David M.
