Memorandum of Agreement (MOA) <br />SRFC Safe Schools/Healthy Students Initiative (SS/HS) Connections Grant <br />Partners of Suburban Ramsey County <br />is to help SS/HS project partners achieve the objectives of the proposed program on time <br />and within budget, including meeting their partnership responsibilities, providing staff, <br />and contracted services, and setting milestones for accomplishing project tasks. <br />The SS/HS Core Management Team is responsible for providing direction to the <br />SSMS Project Director and for leading and managing SSMS project goals and <br />collaborative processes. Ked administrators from each school district, a local law <br />enforcement representative, juvenile justice and a coon mental health services <br />representative will meet quarterly with the SS/HS Project Director to guide, facilitate <br />and promote the goals of the SS/HS grant. (See attached roster of the Core <br />Management Team) <br />The SS/HS Design Team is responsible for designing -in ongoing project <br />planning activities, training sessions, and providing evaluation implementation <br />support using the Casey P.D.S.A. Change Model of (Plan, Do, Study, Act). Key <br />district managers will meet with the SS/HS Project Manager and the SRFC <br />Assistant Director before each SS/HS Core Management Team Meeting to give <br />progress reports and address concerns, problems or barriers affecting the <br />implementation of the comprehensive community wide plan and SS/HS project <br />goals. This team's questions, updates, concerns or emergent needs will be a <br />standard item on the SS/HS Core Management Team's agenda. (See attached <br />roster of the Design Team) <br />At the School Level: <br />o The Superintendents of the four district consortium will be key <br />communicators and coordinators for the day-to-day district -wide SS/HS <br />project and partnership involvement. <br />o The School Building Principals will be key communicators and <br />coordinators for the day-to-day school -wide SS/HS project and partnership <br />involvement. <br />o Each school district will hold regular SS/HS Interdisciplinary Team <br />meetings. SS/HS Design Team members will facilitate these meetings <br />and, with the team's assistance, insure steady progress on the <br />program/project/curriculum and services of the SS/HS project and <br />coordinated services. <br />The attached SS/HS Logic Model, The Governance Flow Chart, the SAT and SS/HS <br />Flow Chart and "Partnership Process of Continuous Improvement" are attached to this <br />MOA for further clarification of the roles, responsibilities and programs of the SS/HS <br />grant. <br />SS/HS Initiative Partners' Roles and Responsibilities: <br />All partners are committed to providing appropriate programs and services to prevent <br />children from becoming at risk and to intervene with children already involved in the <br />juvenile justice system; and <br />Page 9 of 16 <br />
