Memorandum of Agreement (MOA) <br />SRFC Safe Schools/Healthy Students Initiative (SS/HS) Connections Grant <br />Partners of Suburban Ramsey County <br />supervision and monitoring of contractual personnel <br />2. Support school -linked comprehensive mental health services. <br />3. Cross train, via the district wide interdisciplinary team, ways in which all partners, <br />including law enforcement, corrections, and school staff, can better understand all <br />SS/HS elements from a mental health perspective. <br />THE LOCAL EARLY CHILDHOOD MENTAL HEALTH COLLABORATIVE <br />(FOUNDATIONS FOR SUCCESS) AGREES TO: <br />1. Support the comprehensive early childhood mental health continuum of services at <br />school sites and within the community including coordinated standardized screening <br />efforts with designated early childhood programs. <br />2. Support the Foundations for Success ongoing partnership, resource and early <br />childhood mental health staff development in Suburban Ramsey schools and <br />community agencies. <br />THE EARLY CHILDHOOD INFORMATIONAND REFERRAL OF RAMSEY <br />COUNTYAGREES TO: <br />1. Support the comprehensive early childhood mental health triage and follow up <br />services for parents whose children, ages 0-5 had a clinically significant score on the <br />ASQ-SE. <br />TERMS OFAGREEMENT: <br />This final Memorandum of Agreement shall be in effect as of the date on this agreement <br />and valid until September, 2011, unless otherwise modified. All parties are signatory to <br />this agreement when signing. Any party signatory to this agreement may terminate <br />participation upon thirty days notice to all other signed parties to the agreement. <br />A GENC Y R EPR ESENTA TI VES: <br />The parties will develop procedures for ongoing meetings with the SSMS Project <br />Director. <br />MODIFICATION OF AGREEMENT: <br />Modification of this agreement shall be made only by consent of the majority of the <br />initiating parties. Such shall be made with the same formalities as were followed in this <br />agreement and shall include a written document setting forth the modifications, signed by <br />all the consenting parties. <br />OTHER INTERAGENCYAGREEMENTS: <br />All parties to this agreement acknowledge that this agreement does not preclude or <br />preempt each of the agencies individually entering into an agreement with one or more <br />parties to this agreement. Such agreements shall not nullify the force and effect of this <br />agreement. This agreement does not remove any other obligations imposed by law to <br />share information with other agencies. <br />Page 13 of 16 <br />
