IbOf Report Number 08-312 <br />NM Agenda Section VI -3 <br />BRKA-HUN Council Meeting Date November 25, 2008 <br />the city that works for you <br />REQUEST FOR COUNCIL CONSIDERATION <br />ITEM DESCRIPTION: CONSIDERATION OF PARTIAL PAYMENT 13 AND CHANGE <br />ORDER NO.5 FOR PROJECT 07-3, OLD HIGHWAY 8 NW <br />RECONSTRUCTION <br />DEPARTMENT HEAD'S APPROVAL: f m-,� <br />CITY MANAGER'S APPROVAL.- <br />No <br />PPROVAL:No comments to supplement this report ic;�Jr Comments attached <br />Recommendation: To approve contract Partial Payment 13 in the amount of $274,304.74 and Change <br />Order No. 5 in the amount of $9,962.22 for Project 07-3, Old Highway 8 NW Reconstruction to North <br />Pine Aggregate, Inc. <br />Legislative History: Project 07-3 consists of the reconstruction of Old Highway 8 NW from I-694 to <br />the Long Lake Regional Park entrance. The City Council awarded a contract to North Pine Aggregate <br />on June 12, 2007. <br />Financial Impact: Please see attached payment breakdown for details. <br />Explanation: Construction work on Old Highway 8 has been suspended for the winter. Work <br />remaining to be done next spring to final the project includes the remaining street lighting, landscaping, <br />sidewalk, and final paving and lane striping. <br />Change Order 5 adds five items to the contract. The first item involves the hauling and disposal of the <br />contaminated soil that was excavated and paid for under Change Order 4. This work should be eligible <br />for grant reimbursement as part of the previous $2M State Appropriation for Sanitary Sewer and Storm <br />Water Drainage Improvements. Items 2 through 5 are additional watermain items not previously <br />identified and therefore not included in the original bid package. The watermain items will be paid for <br />from the Water System Improvements fund. The total amount of Change Order 5 is $9,962.22. Please <br />see the attached Change Order 5 for a description and cost breakdown of these items. <br />Costs and Funding: The construction portion of this project is being funded from a combination of <br />funding sources including a Mn/DOT grant, EPA grants, Municipal State Aid funds, Ramsey County <br />Cooperative Agreement funds, City Utility funds, and TIF Bond proceeds as outlined in the Feasibility <br />Report. Change Order 4 work will be submitted for reimbursement as part of the State Appropriation <br />for Sanitary Sewer and Storm Water Drainage Improvements. <br />4, <br />Grant M.
