ENGINEERING DEPARTMENT 7326 WYOMING TRAIL CHISAGO CITY,MN 55013 OCTOBER 20,2011 PHONE:(651)257-6864 FAX:(651)257-3393 CHANGE ORDER 2 PROJECT 11-3,2011 PAVEMENT MANAGEMENT PROGRAM&PROJECT 11-8, NEW BRIGHTON COMMUNITY CENTER PARKING LOT PURPOSE/DESCRIPTION: This work order is to add storm sewer improvements at Silver Oaks Park and remove construction of the basketball court The proposed storm sewer construction and basketball court removal was authorized at the September 13,2011 City Council Meeting. Work on the storm sewer includes removal of existing concrete curb and bituminous,and replacement with new materials along with new catch basins over the existing storm sewer pipe. This work order also addresses additional work at Hansen Park The contractor encontoured peat during the installation of the new storm sewer line,and they were instructed to remove it. Additionally,the existing storm sewer manhole(constructed of block)at the intersection of 5th Avenue NW and the entrance to the park was replaced with a 60-inch precast structure(based on its poor condition).This work was not anticipated at the time of the bid and would need to be paid for by change order.
