Standard Grant Template Version 1.2 <br /> Grant Agreement Number <br /> Between the Minnesota Department of Health and The City of New Brighton <br /> • Submission of monthly reports of water production, fluoride chemical consumption, and distribution <br /> system fluoride concentration to the MDH; <br /> • Quarterly collection and submission of a distribution system sample to the MDH Public Health <br /> Laboratory (PHL); <br /> • Daily (or weekly if using the MDH interim policy) distribution system samples must be collected at <br /> locations representative of the entire water supply. Quarterly distribution samples must be collected at <br /> locations representative of any portion(s) of the water supply affected by any improved fluoridation <br /> process equipment during the contract period. <br /> Recipients of funds intended for fluoridation process equipment must submit and receive approval of <br /> plans and specifications from the MDH Drinking Water Protection Section prior to installation. Process <br /> improvement plans and specifications for chemical feed equipment, e.g. storage tanks, chemical feed <br /> pumps, spill containment, or feed tubing, must be provided by either the chemical company that will <br /> supply the chemical feed equipment, or by a Minnesota-licensed professional engineer. The submission <br /> must identify the location of chemical feed equipment and injectors; include equipment manufacturer's <br /> pamphlets; be signed and dated on all drawing and specification pages by a chemical company <br /> representative; be readable; comply with applicable installation rules. No fluoridation process equipment <br /> is to be purchased or construction is to take place until the installation and/or construction plans have been <br /> reviewed and approved by the MDH Drinking Water Protection Section and a grant agreement signed <br /> with the MDH. <br /> Plans and specifications must be submitted to David Rindal, Public Health Engineer, at one of the <br /> addresses in the table below: <br /> Street Address (hand or courier delivery) U.S. Postal Service Mailing Address <br /> Minnesota Department of Health Minnesota Department of Health <br /> Drinking Water Protection Section Drinking Water Protection Section <br /> 625 Robert Street North P.O. Box 64975 <br /> Saint Paul, MN 55155-2538 Saint Paul, MN 55164-0975 <br /> There is no applicable plan review fee for projects awarded Community Fluoridation grants. <br /> Due to possible future changes to recommended and/or mandatory fluoride content at Minnesota <br /> municipal public water supplies, consideration should be given to the range of chemical feed rates <br /> possible from any installed metering pumps. Funded pumps should have the capability to achieve optimal <br /> fluoride content (0.9 to 1.5 mg/L) while allowing for the possible need for turn-down to a concentration <br /> between 0.6 and 1.0 mg/L. Most pumps perform most accurately near mid-range of the stroke length and <br /> stroke frequency and should be selected accordingly. <br /> Contact the MDH District Engineer upon installation/start-up for on-site inspection and water sample <br /> collection <br /> Reporting Requirements: <br /> • The results of performing the work that is described in the grant application; <br /> • The total amount of grant funds that were expended to perform application work item; <br /> Page 3 of 12 <br />
