Christian Science practitioners listed with the First Church of Christ, Scientist in <br /> Boston, Massachusetts; or <br /> Any health care provider recognized by the employer or the employer's group <br /> health plan benefits manager; or <br /> "Child" means a person under 18 years of age (or a person incapable of self-care <br /> because of a physical or mental disability) who is a biological, adopted, foster or <br /> step child, a ward of the employee, or a person for whom the employee is <br /> standing in loco parentis; or <br /> "Parent" includes a biological parent of the employee or a person who previously <br /> acted in loco parentis of the employee, but does not include "parents-in-law"; or <br /> "Care" includes the provision of psychological as well as physical care. It also <br /> includes acquiring care and sharing care duties. <br /> 16.9 Definition of Covered Servicemember <br /> A covered servicemember is: <br /> (1) a current member of the Armed Forces, including a member of the National <br /> Guard or Reserves, who is undergoing medical treatment, recuperation or <br /> therapy, is otherwise in outpatient status, or is otherwise on the temporary <br /> disability retired list, for a serious injury or illness; or <br /> (2) a covered veteran who is undergoing medical treatment, recuperation, or <br /> therapy for a serious injury or illness. "Covered veteran" means an individual <br /> who was a member of the Armed Forces, including a member of the National <br /> Guard or Reserves, and was discharged or released under conditions other than <br /> dishonorable at any time during the five-year period prior to the first date the <br /> eligible employee takes FMLA leave to care for the covered veteran. <br /> 16.10 FMLA Leave Policy <br /> The length of FMLA leave is not to exceed twelve (12) weeks in any rolling <br /> twelve (12) month period with the exception of leave to care for family members <br /> injured while on active military duty which may extend up to 26 weeks. FMLA <br /> leave shall be taken simultaneously with the Minnesota Parenting Leave and <br /> entitlement to FMLA leave for the birth or placement of a child can begin at any <br /> time, but expires twelve (12) months after the birth or placement of the child. <br /> Employees must submit their FMLA leave form thirty (30) days before the leave <br /> occurs unless the nature of the leave is due to an unanticipated event or <br /> emergency. A FMLA leave form is available on the City's intranet site or by <br /> contacting the Assistant City Manager. The form will include a schedule for using <br /> sick leave, vacation leave, compensatory time, unpaid leave, and revised work <br />
