For the aforementioned situations, the local law enforcement agency must be called <br /> (dial 911 if needed) and first aid must be arranged for anyone who is injured. As soon <br /> as the employee is able, the employee must contact their direct supervisor to report the <br /> accident. If the accident is not specifically identified in the above situations, the <br /> supervisor will determine whether or not the local law enforcement agency should be <br /> called to the scene of the accident to conduct an investigation. <br /> A description of the circumstances (such as date, time, location, and weather <br /> conditions), the name and address of the party(ies) and witnesses involved, and the <br /> name and address of the involved parties insurance carrier should be prepared. <br /> Employees should not make any statements concerning who was responsible for the <br /> accident or statements about circumstances causing the accident, except to the <br /> investigating officer. <br /> In the event a Public Safety Department vehicle (police or fire) is involved in a vehicle <br /> or equipment accident the operator of the vehicle shall immediately stop and render first <br /> (if required) aid to anyone that is in need of assistance. The operator of the vehicle shall <br /> notify the Ramsey County Emergency Communications Center, and shall remain on <br /> scene until released by the responding law enforcement agency. <br /> The operator shall advise his or her Supervisor of the accident at the earliest possible <br /> time. The affected employee and supervisor shall jointly complete a City of New <br /> Brighton "Employee Accident Report". The employee's supervisor is responsible for <br /> completing the Employee Accident Report and making sure that this form is completed <br /> properly. Supervisors are also required to submit the completed "Employee Accident <br /> Report" to the Finance Department. <br /> An employee operating a vehicle or equipment that is involved in an accident, will be <br /> subject to drug and alcohol testing pursuant to Section 26.3 (Drugs and Alcohol <br /> Testing) of the Employee Handbook. <br /> Employees operating DOT licensed vehicles may be subject to other testing <br /> requirements in the event of an accident. Employees who operate a DOT licensed <br /> vehicle and are involved in an accident must follow the procedures stated in the City's <br /> Federal Omnibus Transportation Employee Testing Act policy which is located in the <br /> Public Works Department's Standard Operating Procedure Manual. <br /> Personal Injury- (Revised November 13, 2012) <br /> All employees who get injured on the job should obtain appropriate medical assistance. <br /> Employees should report their injury status to their immediate supervisor. The <br /> employee's supervisor or designee will be responsible for making sure that the <br /> employee completes the "First Report of Injury" as soon as possible. The immediate <br /> supervisor is also responsible for completing a "Supervisory Report of Accident" form <br /> which is then attached to the "First Report of Injury" form. Once the injury reports have <br /> been reviewed and initialed by the Department Director or designee in the upper left (or <br /> right) hand corner of the document they will be forwarded to the Finance Department <br />
