<br /> <br /> 8 <br />4.1A Public information about an individual may be shown or given to anyone for any <br />reason. <br /> <br /> 4.1B Private information about an individual may be shown or given to: <br /> <br />4.1B1 The individual, but only once every six months, unless a dispute <br />has arisen or additional data has been collected or created. <br /> <br />4.1B2 A person who has been given access by the express written <br />consent of the data subject. (Exhibit 5) <br /> <br />4.1B3 People, or another responsible authority, which are authorized <br />access by federal or state law or by court order. <br /> <br />4.1B4 People within the City staff, the City Council, and outside agents <br />(such as attorneys, insurance agents, etc.) whose work assignments or <br />responsibilities reasonably require access. <br /> <br />4.1C Confidential information may not be given to the subject of the data, but may <br />be given or shown to: <br /> <br />4.1C1 People who are authorized access by federal or state law or by <br />court order. <br /> <br />4.1C2 People within the City staff, the City Council, and outside agents <br />(such as attorneys, insurance agents, etc.) whose work assignments or <br />responsibilities reasonably require access. <br /> <br />4.2 Form of Request. Any individual may request data verbally or in writing. Data will be <br />released depending on whether or not the City has stored the data requested and whether the <br />data is classified as public, private, or confidential. <br /> <br />4.3 Requests for Private or Confidential Information. All requests to see private or confidential <br />information must be made in writing in order to verify identity. The Responsible Authority will <br />provide a form (Exhibit 3) to document the requesting party’s identity, the information <br />requested, and the City’s response; however, any individual may request data verbally or in <br />writing as long as the request is accompanied by documentation providing the requesting <br />party’s identity and a detailed description of the information requested. <br /> <br />4.4 Identification of Requesting Party. The Responsible Authority must verify the identity of <br />the requesting party as a person entitled to access when private or confidential data is <br />requested. Identity can be established through personal knowledge, presentation of photo <br />identification, comparison of the data subject’s signature on a consent form with the person’s <br />signature in City records, or other reasonable means. <br /> <br />4.5 Time Limits. Requests will be received and processed only at New Brighton City Hall or the <br />New Brighton Public Safety Department during normal business hours.
