.

.

.

Using the Form
The SRTS Grant Application is meant to be deployed electronically. It can be accessed on the
Minnesota SRTS Website, or on CD or in hardcopy by request. The SRTS Grant Application is a
structured MS Word form that allows the user to input information into form fields.

The applicant obtains the Grant Application Instruction Package, and the Grant Application
Worksheet from SRTS website or on disk by request. The Grant Application Worksheet should be
renamed and saved locally, completed as directed, and e-mailed back to the SRTS Coordinator
Kristie.Billiar@dot.state.mn.us. Where possible, subsequent documentation (maps, images, photos)
should also be sent as attachments to the e-mail. The sections to be completed and the amount of
additional documentation are dependent on the overall breadth and scope of the project.

A printer-friendly version of the Grant Application Worksheet is available, although the electronic
version is preferred.

All applications must be received by 4:30 PM COT July 6, 2006. Any applications or supporting
materials received after the deadline will be automatically disqualified.

Multi~School Applications: It is permissible to apply for 1 grant to cover several schools. However,
if it is an infrastructure grant the schools must be in close proximity of one another. This means that
the schools must share or have adjacent campuses, neighborhoods, or blacks. Non-Infrastructure
projects do not have a proximity requirement; however, they should have similar student
demographics.

The remainder of this document consists of the sections to be completed by the applicant, in addition
to the worksheet. Fallowing the sections are appendixes intended to clarify the process. As stated
above, it's preferable that the completed sections, and the worksheet be e-mailed electronically back
to the SRTS coordinator. US mail is also acceptable provided it is received no later than 4:30 PM
COT July 6, 2006.

The application consists of eight sections; if you're applying for an Infrastructure Grant the Applicant
must complete the Grant Application Worksheet and all sections of the application. Non-
Infrastructure applicants are required to fill out the Worksheet, and sections 1, 2, 4, 5, 6B, 7B and 8
only.

Outline of the sections and Appendixes:
. Section 1: Project Area Maps
. Section 2: Description of Current Biking and Walking Conditions in the Project Area and
Demonstration of Need
. Section 3: Describe Proposed Infrastructure Improvement(s)
. Section 4: Describe Proposed Non-Infrastructure Program(s)
. Section 5: Stakeholder Concurrence and Support, and Additional Funding Sources
. Section 6: Project Timeline
. Section 7: Estimated Budget
. Section 8: Application Signature(s)

. Appendix A - Overview of Federal Requirements far using Safe Routes to School Funds
.
