<br />C. Equipment Fees will be .charged on a negotiable basis <br />depending on the type of equipment requested.. <br />D. Deposits <br />1. Groups using the facility during non-office hours will <br />be required a $10.A0 deposit for the use of the key.. <br />2. Groups requesting special equipment or beer permits will <br />also be required a deposit which is set by the actual <br />usage.. <br />Guidelines For 'Facility Use Application <br />1. All groups should request space for .their activity at least <br />one week in advance through the Park. and Recreation <br />Department. Any fees and deposits must be paid at least <br />one working day in advance of usage. <br />2. All groups must have adult supervision and conform to all <br />relevant ,city ordinances and will be issued a permit when <br />request .is granted. <br />3. Permits may be requested for one time usage or for a series <br />of dates.. Permits requested for a series of meeting .dates <br />must be scheduled within our seasonal periods bf Winter: <br />January.- March 15, Spring: March l6 - June 1, Summer: <br />-June 2 - August 31 and Fall: September 1 - December 3-l.. <br />4. Permits granting the use of the clubhouse as requested <br />in the application will be limited to the terms of the permit <br />(time, facility and eq~u.ipment) and only-to the parties to <br />.whom the permit was originally"issued. Cancellations. <br />should be made at least one working day in advance. <br />5. The Park & Recreation Department reserves the right to <br />reschedule an activity if necessary to accommodate requests. <br />User Responsibilities <br />The acceptance of a permit issued for use of the facilities <br />constitutes an acknowledgement by the group of the following <br />conditions <br />1. An adult group supervisor must be present from time of <br />entry until time of departure.. <br />2. The responsibility and liability for injury to persons <br />or damage to or loss of property must be assumed by the <br />organization and/or the individual responsible for .making <br />the application.- Proof of liability insurance coverage <br />may be required. <br />3. Groups :are responsible for their own set-up, take down <br />and clean-up of the rooms. <br />4. All city codes and ordinances must be in compliance <br />by the .group (Sanitation, Fire, Sales,License, Use <br />of alcohol, etc.) <br />5. Use of posters, advertising materials, and"or decorations <br />is prohibited without specific approval. <br />
