<br />INSTRUCTION SHEET <br /> <br />J. li~::-:: should submit the following additional documentation. <br />licants are required to submit those listed: <br /> <br />I. Submit a floor plan diagram of the salon, drawn to 1/4 inch = 1 foot <br />scale, on 8 1/2" X II" or 8 1/2" X 1411 paper, providing dimensions as <br />a whole and designating the size and location of all entrances and <br />exits, and indicating the location and dimensions of all required <br />areas, facilities and equipment. (These requirements are listed in <br />4 MCAR S 10.113 - 4 MCAR S 114 for salons.) See Sample Floor Plan <br />attached. <br /> <br />Salon <br /> <br />2. If your beauty salon is located in a rural area, you are required to <br />send a diagram with instructions stating:. Nearest town, County road <br />or highway number, Landmarks and Husband's name. See Sample Location <br />Diagram attached. <br /> <br />3. Salon applicants submit evidence of coverage by professional liability <br />insurance of at least $25,000 for each claim and $50,000 total <br />coverage for each policy year for each operator. <br /> <br />I <br /> <br />REQUiREMENTS FOR <br /> <br />4. <br /> <br />1. <br /> <br />2. <br /> <br />3 . <br /> <br />4 . <br /> <br />5. <br /> <br />, <br /> <br />8. <br /> <br />Salon applicants must provide documentation of workers compensation <br />insurance by submitting evidence of compliance with Minnesota Statute <br />S 176.182. <br /> <br />Submit $50.00 for a 3 year license and $15.00 processing fee. Total <br />$65.00 FEE. Make checks payable to TREASURER, STATE OF MINNESOTA. <br /> <br />SALON LICENSURE. <br /> <br />Upon .pplication to the office a provisional license will be issued <br />which will enable the salon to open for business. <br /> <br />After Inspection by the office staff, a permanent license for the <br />balance of the 3 year license cycle shall be issued upon confirmation <br />of the salon's compliance. <br /> <br />There shall be at least 120 square feet of work space exclusive of any <br />restroom, reception, or supply area for a one practitioner cosmetology <br />salon, and 100 square feet of work space for a one practitioner <br />manicure salon. <br /> <br />There shall be at least 50 additional square feet of work space for <br />each additional licensee simultaneously on duty in a cosmetology <br />salon, and at least 35 additional square feet of work space for each <br />additional lice~see simultaneously on duty in a manicure salon. <br /> <br />Each salon shall have at least two entrance/exit points, which comply <br />with State fire codes. <br /> <br />The supply area shall not be accessible to the public. <br /> <br />There shall be clean, hot, and sanitary running water provided in <br />the dispensary. <br /> <br />Each salon shall provide adequate toilet facilities on its premises. <br /> <br />-OVER- <br />
