<br />-2- <br />Requireme~t~ F?r Sal?n Licensure, Cant. <br /> <br />._~. <br /> <br />9. Salon ventilation sl~all comply with the requirements of the State <br />Building Code. Window ventilation shall not be acceptable as method <br />of meeting the standard. <br /> <br />10. There shall be a waste receptacle at each work station. It shall be <br />lined with a disposabl~ plastic bag or liner. or washed daily. <br /> <br />11. There shall be at least one covered metal container which shall be <br />used exclusively as a receptacle for soiled towels and linen. It <br />shall be kept closed at all times. <br /> <br />12. There shall be at least one large covered metal container for disposal <br />of garbage. <br /> <br />13. There shall be a storage cabinet for all clean linen and towels. <br /> <br />14. Each salon shall maintain a readily accessible first aid kit. <br /> <br />15. Each salon shall have at least one readily accessible fire extinguisher <br />which complies with state fire code requirements. <br /> <br />SALONS IN PRIVATE RESIDENCES. SALONS MAY BE ESTABLISHED IN PRIVATE RESIDENCE <br />IF THE FOLLOWING CONDITIONS ARE MET: <br /> <br />'~. The salon shall not be used for any residential purposes at any time; <br />2. The salon shall be completely separated from the residential areas. <br />There shall be a permanent. solid partition. from floor to ceiling. <br />between the salon and the residential areas. Any door in this <br />partition shall be kept closed at all times. <br />3. All cosmetology services offered by the salon shall be available <br />within the salon. <br />4. Laundry tubs shall not be used as shampoo bowls. <br />5. The primary residential toilet facilities shall not be used for salon <br />clients. <br /> <br />BOOTH LICENSE APPLICANTS MUST SUBMIT THE FOLLOWING DOCUMENTATION: <br /> <br />1. <br /> <br />Documentation of holding a current Minnesota Cosmetologist. Manager <br />license or Manicurist/Manager license. <br />Documentation of leased work space from a licensed Minnesota salon with <br />work space of at least 50 sq. ft. for a cosmetologist or 35 sq. ft. for <br />a manicurist. The lease shall include provisions regarding <br />responsibility for keeping records of hours worked by the booth <br />licensee. maintenance responsibilities of the booth licensee. use of <br />salon equipment. purchase of supplies, professional liability coverage. <br />and other agreements reached by the parties. This agreement may also <br />state that the booth operator is an independent contractor and is free <br />to set prices and work hours independent of the salon; is free to <br />purchase supplies from whomever desired; and is free from extra duties <br />such as telephone coverage. sweeping and cleaning. etc.; except for <br />the booth operation proper. <br /> <br />2 . <br /> <br />3 . <br /> <br />Diagram of Booth Area. drawn to scale 1/4"= I foot. <br /> <br />I <br /> <br /> <br />1.. <br /> <br /> <br />'. <br />
