<br />I . <br />I <br />I <br />i <br />i <br />I Pay checks shall not be issued other than on regular paydays. <br />Employees who will be absent on regular paydays due to vacation, <br />sick leave or leave of absence ('HId wish to receive their checks <br />will be required to leavo a self-addressed, stamped envelope with <br />the finance director. (Code 1966 S 240.18(6); Ord. No. 407 S I, <br />12-27-77) <br /> <br />Section 21-13. Performance Evaluation <br /> <br />i <br />, There shall be a minimum of one written performance review per <br />year for all regular and probationary employees. Performance <br />" evaluation forms will be completed by the employee's immediate <br />supervisor. A personal interview between the employee and the <br />I immediate supervisor shall be conducted within a reasonable <br />I period of time after receipt by the employee of the written <br />I evaluation. <br /> <br />I <br />I <br />I <br />I <br />\ <br />I <br />I <br />I <br />I <br />I <br /> <br />The evaluation will include a review of the employee's principal <br />responsibilities, an appraisal of the employee's job performance, <br />a discussion of problem areas, and a plan of action to set Objec- <br />tives for performance and to assist in alleviating any problem <br />areas. <br /> <br />All evaluations shall be submitted to the City Manager for revie <br />and be made a part of the employee's personnel records. When an <br />employee is eligible for a pay step increase, the evaluation <br />required in this section shall be coordinated with the evaluation <br />required in the Compensation section of the Administrative Code. <br />(C od e 1966, ~ 240. 47; 0 rd. No. .3 4 7 . 1- 28 - 7 5) . <br /> <br />Section 21-14. Physical ExamiQations <br /> <br />Employees shall be medically and psychologically fit for a posi- <br />tion they occupy in the City. Symptoms of physical illness or <br />psychological problems will form the basis for requesting an <br />examination at the discretion of the City Manager. If a physicia <br />or psychologist certifies that the applicant or employee is not <br />able to perform the duties of the position, the employee may be <br />transferred to a position for which the employee is determined by <br />the City to be qualified, provided such a position is vacant. <br /> <br />If such employee is unable to qualify for a position in an exist <br />Ing classification, or if no position is vacant, such employe <br />shall be discharged. (Code 1966 ~ 240.50; Ord. No. 347, 1-28-75) <br /> <br />Section 21-15. Disciplinary Actions <br /> <br />Employees shall be subject to disciplinary action for failing t <br />fulfill their duties and responsibilities. It shall be the polic <br />of the City to administer disciplinary action uniformly and with <br />out discrimination. Disciplinary action may include action up t <br />and including termination. Discipline will be based upon th <br />nature and severity of the infraction and conditions surroundin <br />the incident using generally accepted principles of progressiv <br />discipline where possible. <br /> <br /> <br />,. <br />I <br /> <br />-7- <br />
