her discretion, but shall be allowed in case of necessity and actual illness, legal <br />quarantine, or disability of the employee or to receive dental or medical care or other <br />sickness preventative measures approved by the City Manager. <br />25.4 Use for Funerals/Illness in Family. Employees may use up to three (3) days of paid <br />funeral leave to attend the funeral of a member of the employee's immediate family <br />(see definition of immediate family in Section 3 of the City's Employee Handbook). Up <br />to two (2) additional days of sick leave may also be used for attending the funeral of an <br />employee's immediate family member. <br />Up to three days of sick leave may also be used for the purpose of attending to the <br />needs of a critically ill member of the employee's immediate family. Critically ill is <br />defined where a physician has indicated that there is the imminent probability of death <br />or that the employee is ordered to observe and monitor the condition of the health of the <br />employee's kin. The period of sick leave usage may be extended in 30 calendar day <br />increments, up to 90 calendar days, if pre -approved by the City Manager. <br />25.5 Notice. In order to be eligible for sick leave with pay an employee must notify the on - <br />duty supervisor or department head not less than two (2) hours prior to the beginning of <br />the employee's work day, and shall keep the department head informed of the <br />employee's condition if the absence is more than three days duration. <br />25.6 Substantiation. Proof to support sick leave shall be required as follows: <br />A. An attending doctor's statement may be required for any sick leave taken; <br />B. An employee claiming sick leave may be required to file competent written <br />evidence that the employee has been absent as authorized. Sick leave taken <br />immediately preceding termination of employment or retirement of an employee <br />shall always be substantiated by a written medical report; <br />C. An employee who has been unable to work for a period of time because of illness <br />or accident, may be required, before being permitted to return to work, to provide <br />medical evidence that the employee is again able to perform all significant duties <br />of the job in a competent manner and without hazard to the employee or others. <br />D. The City Manager may require a nurse visitation or medical examination, or both, <br />as deemed necessary before or during an approved utilization of sick leave. Any <br />nurse visitation or required examination shall be paid by the City. <br />25.7 When Denied. No sick leave shall be allowed for illness, injury or physical inability <br />resulting from misconduct on the job except as provided in Section 15 (Sick/Funeral <br />Leave) of Employee Handbook. No additional time off will be allowed for any holidays <br />occurring during an employee's sick leave account. No sick leave benefits of any kind <br />shall be granted after the termination of employment. <br />15 <br />
