All City employees participating in an NBCC Employee Membership benefit are expected to <br />follow all NBCC rules and policies including those stated on the enrollment form. <br />Benefitted Employees: All benefitted employees, including full-time, regular part-time, paid - <br />on -call firefighters and elected officials, electing to participate in this benefit must complete an <br />Employee Membership form available through the New Brighton Community Center. This form <br />is then approved by the Department Director in order to verify employment and track budget <br />expenses. <br />All annual memberships will be effective for one ,year from the date of enrollment. Near the <br />expiration date, an email will be sent to the employee with instructions on how to renew their <br />annual membership. Once a completed form is on file, the employee is just responsible to <br />complete the name, phone number and Department Director signature on a subsequent <br />membership forms, unless there are changes to their family status. Employees are required to <br />sign the form as this indicates they have given the City_ permission to deduct the taxable income <br />from their paycheck. Spouses and children are not required to sign the form. <br />Once the membership has been made active at the NBCC, paperwork is sent to the Finance <br />Department as record for the payroll deduction. Depending on timing, the deduction will be <br />made either one or two pay periods after. Taxes collected on this membership are non- <br />refundable. <br />Seasonal/Part-Time Employees: Any seasonal or part-time employee electing to participate in <br />this benefit must complete a Seasonal Employee Membership form. Any membership category <br />is eligible for a 20% discount, including annual or monthly_payment options. If the monthly <br />payment option is chosen, the employee must stay an active member for 90 days. After this, the <br />membership will reoccur until a cancellation notice is acquired. <br />Employment status must be verified prior to the 20% discount being accessed. <br />5.5 Personal Training Services <br />Benefitted Employees: Any benefitted employee, including full-time, regular part-time, paid - <br />on -call firefighters and elected officials are also eligible to receive up to six hours of personal <br />training services per calendar year at no cost to them. These services are arranged and tracked <br />through the NBCC certified personal trainers. Employees are encouraged to take advantage of <br />this benefit even if they chose not to enroll as a member of the NBCC. This is a great way to for <br />employees to get a base line measurement, get introduced to different equipment or get set up on <br />a work out plan. <br />Additional personal training services/packages above and beyond the six hours can be purchased <br />at the employee's expense. This benefit is ONLY available to the City of New Brighton <br />employee, no spouses, partners or children are eli jhjl . <br />5.6 Group Fitness Classes <br />
