Calls or events categorized by the Public Safety Department: miscellaneous juvenile status crimes, 404 <br />liquor offenses or curfew violations, disturbing the peace or harassing communications, property 405 <br />damage, criminal damage to property or trespass, domestic incidents, fire alarms, public 406 <br />disturbance or disorderly conduct, loud party or noise complaints, disorderly juveniles, assault in 407 <br />the fifth degree, or non-domestic related assaults. The Director of Public Safety shall maintain for 408 <br />public inspection a description of the coding system and a list of the codes and crimes included 409 <br />within each of these categories of calls or events. 410 <br />4. The Director of Public Safety may use discretion to determine that multiple incidents be counted as 411 <br />a single call in appropriate cases. 412 <br />(B) Calls will not be counted for purposes of triggering a provisional license where the victim and suspect 413 <br />are “Family or household members” as defined in the Domestic Abuse Act, Minnesota Statutes, Section 414 <br />518B.01, Subdivision 2(b) and where there is a report of “Domestic Abuse” as defined in the Domestic 415 <br />Abuse Act, Minnesota Statutes, Section 518B.01, Subdivision 2(a). 416 <br />(C) The periods of time used to determine whether a provisional license is required shall be the six month 417 <br />period between January 1st and June 30th of each calendar year, and the six month period between July 418 <br />1st and December 31st of each calendar year. 419 <br />(D) The Public Safety Department will provide, by mail, a monthly status report of police and fire calls 420 <br />counting towards provisional status to all license holders. 421 <br />(2) Action Required. Upon being notified by the Public Safety Department that provisional license status has 422 <br />been triggered, the licensee shall submit an application for provisional license status within seven (7) 423 <br />calendar days, or by a specific deadline established by the Public Safety Department. Failure to apply for 424 <br />provisional license status by the applicable deadline will be grounds for revocation or suspension of the 425 <br />license pursuant to the provisions of Section 13-158. 426 <br />(3) Provisional License Application. Applications for a provisional rental license shall be submitted on the 427 <br />appropriate official application form identified and provided by the city. At a minimum, applications must 428 <br />include the following to be considered complete: 429 <br />(A) The name, address, telephone number, 24-hour cell number (if different), e-mail address, and date of 430 <br />birth of the property owner seeking the provisional rental license. If the owner identified in the 431 <br />application is a legal entity, the applicant shall submit the name, address, e-mail address, and phone 432 <br />number for all partners, shareholders, or interest holders. 433 <br />(B) The name, address, telephone number, 24-hour cell number (if different), e-mail address, and date of 434 <br />birth of a designated local manager who resides on the property or at a location within the Twin Cities 435 <br />metropolitan service area, who is on-site or is available 24 hours a day, and who is willing and able to 436 <br />go through required manager certification. 437
