School Facility Usage Responsibilities <br />'he acceptance of a permit issueQ for the use of schocl facilities constitutes an <br />ackncrlclg_rnent by Lhe organization or group of the following conditions: <br />A. An adul*_ gmu,^ supervisor ;permit holder) must be present from the time of <br />entry irttc the facility until the time of departure. The leader must identify <br />him/herse,f to the Buit6ing Supervisor present in the building. <br />B. ;he responsibility and liability for. injury tc persons or damage tc property <br />must be assumed 5y the organization and,!or the fndividual responsible far <br />making the application. <br />C. Groups are responsible for setting up and returning rooas anQ equipment used <br />to the same location and arrangement they rare in prior to usage. <br />0. CanceTiation or Changes: <br />1. A11 building use permits are subject to cancellation if conduct. or <br />infraction of the ruies is warranted. <br />2. If two (2) suc_essive sessions are ~aissed ,vithout 4E hours advanced <br />notification, the penait rill be automatica7Iy cancelled. <br />3.
