I <br /> <br />The total monthly premium for participating employees shall, <br />be paid in accordance with the provisions of the pay Planl <br />resolution. <br /> <br />Date of eligibility for group health and life insurance shalll <br />coincide with the coverage provided by the City. ! <br /> <br />An employee may elect to conti nue group hea lth and li f el <br />insurance coverage while on leave of absence without pay. <br />The entire cost of such insurance shall be paid by the <br />employee in advance of the date payment is due to the <br />insurance carrier. <br /> <br />i <br />I <br /> <br />! <br /> <br />, <br /> <br />I <br />I <br />I <br />I <br />Employees shall be considered to be working for the purposel <br />of accumulating vacation or sick leave when they are usingl <br />earned vacation leave or sick leave, when using injured-on- <br />duty benefits, when on jury duty, when on holiday leave or <br />when on military leave in accordance with Minnesota Statutes, <br />Section 192.26. <br /> <br />An employee may continue any individual or dependent health <br />insurance coverage for a time set by State Statute. The <br />terminating employee shall pay the entire cost of such <br />insurance continuation. (Code 1966, S 240.32; Ord.
