" .,' <br /> <br />Specific reasons for which an unpaid leave of absence will be con- <br />sidered include, but are not limited to: <br /> <br />(a) Chi Id care <br /> <br />(b) Disability incurred while in the service of the City <br /> <br />(c) Military service <br /> <br />(d) Death or serious injury in the immediate family <br /> <br />i Upon expiration of their approved leave without pay, the employee <br />I shall be reinstated to former position or a position of like <br />I status and pay. Failure of the employee to report for work at the <br />I expiration of the leave shall be cause for discharge. , <br /> <br />I Any regular employee shall be entitled to a military leave ofl <br />!!absence without pay for services in the armed forces for a <br />I'jrequired tour of duty as provided for in M.S.A. SI92.26. Request <br />Ifor unpaid military leave shall be accompanied by a copy of the <br />Ilmilitary orders and shall be made prior to the commencement of <br />II said unpaid leave. <br />II <br />Ilsection 21-67. Sick Leave <br /> <br />II (a) Purpose <br /> <br />Sick leave shall not be considered as a privilege or vested <br />right which employees may use at their discretion but shall <br />be considered as a type of insurance which shall be allowed <br />in case of necessity and actual illness, legal quarantine, or <br />disability of the employ~e, or because of death or critical <br />illness in their immediate family, or to receive dental or <br />medical care or other sickness preventative measures approved <br />by the employee's supervisor. Critical illness in this case <br />shall be defined as an illness of such a nature that a physi- <br />cian indicates there is the imminent probability of death or <br />that the employee is ordered by a physician to observe and <br />monitor the condition of the health of the employee's kin. I <br /> <br />(b) Notice <br /> <br />The employee shall request use of sick leave prior to the <br />beginning of the employee's workday. The request shall be <br />made to the employee's immediate supervisor. An employee <br />claiming sick leave may be required to file a physician's <br />certificate or a statement indicating the nature of their <br />illness signed by the employee. <br /> <br />(c) Use for Funerals/Illness i9 Family <br /> <br />When necessary, up to three (3) days of sick leave may be <br />granted to an employee because of death or critical illness <br />of a member of the employee's immediate family. <br /> <br />(d) When Denied <br /> <br />i <br />, <br />No sick leave shall be allowed for illness, injury or physi-! <br />cal inability resulting from misconduct on the job, or ex-'!' <br />cessive use of alcohol or narcotics, except as provided inl <br />section 2l-67(a). No additional time off will be allowed for' <br />any holidays occurring during an employee's sick leave. No <br />sick leave benefits of any kind will be granted after termin- <br />ation of employment. <br /> <br />-14- <br />
